That’s what Care Coordination Services are for. They can help make sure you have the extra care you need to live a healthier and more satisfying life.

How will Care Coordination Services help me?
When you work with Care Coordination Services, you will have one person dedicated to helping you – your own care coordinator. This person’s main job is to make sure you get what you need from your doctors, providers, specialists, community support agencies and social services. Your coordinator will also help you:
- Understand how your health plan works.
- Find a provider who can help you with your special health care needs.
- Get appointments when you need them with your PCP, with specialists or with other providers.
- Get equipment, supplies and services that you need.
Who’s the most important part of your Care Coordination Services team? You are!
Even if you have to see many different providers or get help from many different services, the focus is always on you. Your care coordinator will ensure you’re connected to all the help you need.
If you think you need Care Coordination Services, talk to your primary care provider (PCP). If you don’t have a PCP, call our Customer Service toll-free at 855-722-8208, TTY 711.
Your participation in Care Coordination Services is completely up to you. If you decide you do not want or need these services, you can decline or discontinue them at any time. Stopping will not disrupt the care you are receiving.
