Transitions of care
Are you are switching to Jackson Care Connect from another CCO, or from OHP fee-for-service? We may reach out to help you transition over to your new benefits.
For some members, Jackson Care Connect will receive information from your previous plan, which will help us continue your care.
We will honor written documentation of prior authorization for ongoing services. Items that you are already getting, such as prescriptions or medical supplies, will still be available to you during the transition. Please make an appointment with your new PCP, PCD or mental health provider right away so you can update your treatment plan and keep getting any ongoing care you need.
This transition of care period lasts for:
-30 days for physical health and dental health, and 60 days for mental health;
-OR, until your new providers review your treatment plan.
For members who are dually eligible for Medicaid and Medicare, the transition period lasts for 90 days.
If you try to refill a medication or use previous services and you are told you do not have coverage, please let us know right away, so we can help!
