As part of JCC's commitment to the health of our community, we are investing in community-based strategies that address social determinants of health and equity (SDOHE) and align with the OHA's SHARE initiative and Jackson County’s Community Health Improvement Plan (CHIP) priorities.

Program Overview 

This year’s funding is a joint cycle of CHIP grants and SHARE initiative funding. The Oregon Health Authority’s Supporting Health for All through Reinvestment (SHARE) Initiative requires coordinated care organizations (CCOs) to reinvest a portion of profits in community-based strategies that address social determinants of health and equity (SDOH‑E). This RFP invites competitive proposals for projects to be recommended by JCC’s Community Advisory Council (CAC). Projects that meet SHARE requirements will be prioritized in this cycle.

Eligibility and Alignment 

Projects must meet the following requirements: 

Projects may also address the following requirements, in order to qualify for SHARE funds: 

  • Contribute to the statewide priority of housing‑related services and supports across the SHARE portfolio (project may directly address housing or be part of a balanced portfolio that does). 
  • Address at least one SDOH‑E domain:. 
    • Economic stability 
    • Neighborhood and built environment 
    • Education 
    • Social and community health 
    • Health care access and quality (per October 2025 OHA guidance)

Funding Parameters 

  • Maximum per proposal: $25,000
  • Term: Up to three years to spend down funds

Timeline 

  • March 31, 2026, 5 p.m.: Proposals due 
  • April: Eligibility review and scoring 
  • May 15, 2026: CAC deliberation and funding recommendations complete 
  • June 1, 2026: Award announcements 
  • June 2026 onward: Contract development and execution (if applicable)  
  • August 2026: Fund disbursement begins
