Nutrition is important for overall health because the food we eat provides the energy and nutrients our bodies need to grow, stay strong, and function properly. Eating a balanced diet with fruits, vegetables, whole grains, protein, and healthy fats helps our bodies stay healthy and fight off illness. When we eat the right foods, we feel better, think clearly, and have more energy to do the things we enjoy.

Am I eligible for nutrition supports?
To be eligible for nutrition supports, all four of the following must be true:
- You must be a current member,
- You are not able to get the service through any other benefit or means,
- The item or service must help with your medical need or life situation, and;
- You must be experiencing significant food insecurity.
Not all requested items or services are granted/funded. Our team evaluates each request on a case-by-case basis and makes the final funding determination. Please review our Social Needs Assistance Guidelines for more information about qualifying medical conditions and life situations and the commonly funded supports for each.
How do I request a nutrition support?
If you have met the eligibility standards listed above and want to request nutrition support, simply click the button below to fill out the secure digital request form.

Documents related to your request should be uploaded in advance. Documentation can be emailed to hrsncx@careoregon.org or uploaded to the member portal. If you already have an account go here to login or you can find registration information here to set up an account. Once logged in, upload your documents by doing the following:
- Go to the message center and click "new message"
- Click "add recipients" and select "Health Related Services Requests"
- Add HRSN in the subject line and attach the file(s) to the message using the attach file feature
- Add any other context and click "send"
You can fill out either form yourself, or with the help of a parent, a caregiver, or a guardian/support/trusted friend, or a staff member from an organization or clinic.

Fruit & vegetable and pantry stocking benefits FAQ
- Fruit & vegetable benefit includes minimally processed fruits and vegetables, such as:
- Fresh, frozen, dried, puréed or canned produce
- Herbs
- Pantry stocking benefit includes minimally processed whole foods, such as:
- Fruits and vegetables
- Meat, poultry, fish, plant‑based proteins, legumes
- Dairy
- Whole grains, breads, cereals
- Cooking oils, spices, herbs
- Snacks and non-alcoholic beverages
- Seeds and plants that produce food for the household to eat
You may be eligible if you:
- Are an active OHP member
- Have certain life transitions
- Are experiencing food insecurity
- Have a health condition that meets benefit requirements
- And be able to meet one of the following (for Pantry Stocking):
- Are under age 21
- Are pregnant or have pregnancy-related health risks
- Are a young adult ages 19–26 with special health care needs

Once a decision is made, we will send you a letter informing you of the next steps.

- A prepaid card usable at approved grocery retailers and farmer’s markets

- For the fastest way to submit documents or check the status of your request, please use the secure Jackson Care Connect Member Portal. This online messaging platform is available to members age 18 and older who have internet access through a desktop computer.
