Health Related Services can help fund many other social needs supports for eligible members. If you require a social needs support to help with your health condition and it’s not provided by any other benefit, you can request it here.
Examples of other supports covered
Here are some examples of common support for social needs. This list isn't everything we can help with. If you need something for your health that isn't listed, just ask us and we'll consider your request.
- Sensory support items/tools (weighted blankets, sleep aids, vouchers for yoga classes, etc.)
- Exercise equipment and gym memberships
- Electronic devices such as laptops and communication devices
- Child and safety items
- Specific educational materials and/or classes
- Camp enrollment
- Public transit passes or gas cards
Exclusions
The following supports and services are NOT currently covered:
- Requests for reimbursement
- Items and services that are available under your health plan benefits. Includes, but not limited to:
- copayments
- out-of-network or private provider payments
- partial payments for medical services
- durable medical equipment (DME) (If you have received a bill for medical services, please contact Customer Service at 541-500-0567, toll-free 855-722-8208 or TTY 711)
- Credit card bills or loans
- Mortgage payments
- Fees (pet fees, legal fees, collection, guardianship, etc)
- Payment for caregivers or other hired supports long term
- Payment for non-members
- Such as funding a friend or family member’s utilities or providing a climate device such as an air conditioner when the member is not living at your residence
- Payment related to the construction of new physical structures
- General nutritional supplements
Am I eligible for a social needs support?
To be eligible for a social needs support:
- You must be a current member.
- You are not able to get the item/service through any other benefit or means, and;
- The item or service must help with your medical need or life situation.
Not all requested items or services are granted/funded. Our team evaluates each request on a case-by-case basis and makes the final funding determination. Please review our Social Needs Assistance Guidelines for more information about qualifying medical conditions and life situations and the commonly funded supports for each.

If you have met the eligibility standards listed above and want to request a housing support, simply click the button below to fill out the secure digital Social Needs Request form.

Also, you will need to download and fill out the Health Related Income/Budget Worksheet and submit it with your request.
You can fill out these forms yourself, or with the help of a parent, a caregiver, trusted friend, guardian or a staff member from an organization or clinic.
If you have questions or would like our help filling out the forms, call CareOregon Customer Service at 503-416-4100, or TTY 711. Press option 1 for “member,” then option 6 for “social needs.”

Documents related to your request should be uploaded in advance. They can be emailed to hrsncx@careoregon.org or uploaded to the member portal. If you already have an account go here to login or you can find registration information here to set up an account. Once logged in, upload your documents by doing the following:
- Go to the message center and click "new message"
- Click "add recipients" and select "Health Related Services Requests"
- Add HRSN in the subject line and attach the file(s) to the message using the attach file feature
- Add any other context and click "send"

In addition to Jackson Care Connect’s Health Related Services, there are other local resources that offer social needs assistance in our community. If you find that you do not qualify for our program, you may be able to access what you need from one of the links below.
